A study on the perceptions and behaviors of ICU nursing staff on thirst in mechanically ventilated patients in Guizhou Province

Objective To invstigate the knowledge,attitudes and practices(KAP) of ICU nurses toward thirst in patients with mechanical ventilation in Guizhou Province,and to analyze the influencing factors. Methods The convenience sampling method was used to investigate the knowledge,attitudes and practices of ICU nurses on thirst in patients with mechanical ventilation in 67 hospitals in Guizhou Province. The questionnaire included 38 items in 3 dimensions,namely knowledge,attitudes and practices. Results A total of 392 ICU nurses were surveyed,with the total score of (85.92±13.65),knowledge dimension of(13.42±2.54),attitude dimension of(40.86±7.41) and behavior dimension of(31.65±7.30). Multiple linear regression analysis showed that training,literature learning,hospital grade,age and gender were independent influencing factors of thirst knowledge,attitude and behavior of ICU nurses on mechanical ventilation patients(P<0.001). Conclusion ICU staffs’ knowledge and attitude towards thirsty in patients with mechanical ventilation were positive,but practices need further improvement. To date,there is a need to strengthen the theoretical and clinical practice training of thirst related knowledge among ICU nurses,especially for those working in primary-level hospitals,to improve the internal motivation of nurses to intervene in thirst of patients with mechanical ventilation,so as to further promote the quality of clinical practice and the recovery of patients.

Objective To learn about current status of knowledge,attitude and practice of humanistic care in Chinese nurses and their influencing factors,so as to provide references to the mangers to accordingly improve their knowledge,attitude and practice. Methods From July to August,2022,a general information questionnaire and a clinical nurse questionnaire of knowledge,attitude and practice were sent out and filled by 15 727 nurses from 107 hospitals in 27 provincial administrative regions of China. Results 15,653 nurses completed the investigation. The total score of knowledge-attitude-practice was 84.19±12.98. The scores of knowledge,belief or attitude,and standard score of practice were 80.22±15.61,83.55±15.17,and 87.92±12.70,respectively. The influencing factors were gender,title,department,satisfaction degree with work,emphasis degree of self-care,received training of humanistic care or not,supporting degree from family to their work,relationship with colleagues,satisfaction degree with salary,and having worked or not in a trial ward of humanistic care(P<0.001). Conclusion The overall humanistic care of Chinese nurses currently stands at medium level. The knowledge of humanistic care is relatively inadequate. The managers should strength nurse humanistic care training,give organizational support,build a better department atmosphere,and improve nurses’ satisfactions to their work,salaries,and relationship with colleagues,so as to improve nurses’ humanistic care of knowledge,attitude and practice.

Effectiveness of awake prone positioning in patients with acute hypoxemic respiratory failure:a Meta-analysis

Objective To evaluate the effect of awake prone positioning strategy in patients with acute hypoxemic respiratory failure(AHRF). Methods 7 electric databases including the China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP Database,PubMed,Embase,Web of Science,and Cochrane Library were systematically searched for randomized controlled trials and observations on awake prone positioning in patients with AHRF. The search time limit is from the establishment of the database to August 2022,and the references of the included literature are also traced. 2 researchers independently performed literature screening,data extraction and quality assessment according to the inclusion and exclusion criteria. Meta-analysis was performed by RevMan 5.3 software. Results 12 pieces of the literature were finally included,including 5 randomized controlled trials and 7 cohort studies,with a total of 2 632 patients. The results of Meta-analysis showed that the intubation rate of the awake prone positioning group was significantly different from that of the routine care group[RR=0.73,95%CI (0.65,0.82),P<0.001];compared with those of the routine care group,the differences of the mortality rate[RR=0.87,95%CI(0.74,1.03), P=0.110] and hospital stay[MD=-1.05,95%CI(-4.22, 2.12),P=0.520] were not statistically significant. The secondary adverse effects to awake prone positioning were reported in 7 studies, including discomfort(8%~35%),pain(7%~11%),vomiting(2%~8%),catheter displacement(4%~6%),skin ulceration(0.4%~4.0%),hypotension(4%),venous thromboembolism(2.3%),cough(2%),and aspiration pneumonia(1.5%). The time range of a single prone position of the patients was quite different(1.25~15.00) h. Conclusion Awake prone positioning can effectively reduce the intubation rate of AHRF patients,but it cannot reduce the hospital stay of patients,and the impact on mortality needs further research and verification. Intolerance and pain were the most common adverse reactions when patients were in the prone position,affecting their compliance. Therefore,it is suggested that high-quality research should be carried out in the future,aiming at strengthening nursing management of awake prone positioning,improving patient compliance,and further improving the success rate of treatment.

Extracorporeal membrane oxygenation(ECMO) is used to rescue patients with severe cardiopulmonary failure,and nurses are important members of the ECMO team. This paper introduces the qualification requirements,training content,training methods and qualification accreditation of nurses participating in ECMO monitoring training at home and abroad,and puts forward relevant suggestions based on the current situation in China,in order to provide references for carrying out ECMO monitoring training in China and provide the guidance for managers to formulate relevant policies.

Analysis of potential categories and influencing factors of fatigue trajectories in post-intensive care syndrome patients

Objective To explore the potential trajectory categories of fatigue degree in patients with post-intensive care syndrome(PICS),and to analyze the influence of patient demographic data and disease-related data on the potential categories of fatigue trajectory. Methods A convenient sampling method was used to select patients hospitalized in general ICU ward of a tertiary hospital in Guizhou Province from January 2020 to September,and the demographic and disease-related data of patients was collected. The fatigue degree of the patients was evaluated at 1 week(T1),1 month(T2) and 3 months(T3) after ICU discharge. The growth mixture model was used to identify the potential trajectory category. Logistic regression analysis was used to analyze the prediction factors of trajectory category. Results 556 research subjects were initially enrolled,and 300 of them developed PICS. 4 types of fatigue trajectories were identified in these patients,and they were sustained fatigue group(19.00%),increased fatigue group(6.00%),fatigue remission group(27.67%)and non-fatigue group(47.33%). Logistic regression analysis showed that high APACHE Ⅱ score,advanced age and history of respiratory diseases were independent predictors of fatigue trajectory categories. Conclusion The degree of fatigue in patients with PICS shows different trajectories. Medical staff should pay attention to the evaluation and intervention of fatigue in patients with high APACHE Ⅱ scores,advanced age,and history of respiratory diseases.

ICU acquired weakness affects the recovery of critically ill patients and the quality of life after discharge. Early functional exercise can reduce the incidence of ICU acquired weakness and promote the recovery of patients. In this article,the necessity of early functional exercise for ICU patients is described firstly. Secondly,the assessment of early functional exercise is also summarized,including the specific contents of the initial and continuous assessment of early functional exercise. Besides,the current status and problems of the research on the implementation of early functional exercise,including progressive functional exercise and goal-oriented functional exercise,are reviewed. Lastly,the impediments to the implementation of early functional exercise are analyzed,and the corresponding countermeasures are proposed. The purpose of this study is to provide a reference for clinical and nursing staff to implement early functional exercise effectively and safely.

Latent class analysis of symptom characteristics in patients with post-intensive care syndrome

Objective To explore the latent classes of symptom characteristics of post-intensive care syndrome(PICS)patients and to analyze the differences in demographic and disease characteristics of patients among different classes. Methods According to the inclusion and exclusion criteria,patients in the general ICU of a tertiary hospital in Guizhou were selected as the research subjects from August 16,2019 to January 16,2020,and from June 8,2020 to October 30,2020 by using convenient sampling method. Patients were followed up by telephone after being transferred out of ICU for 1 month to assess their PICS-related symptoms. Latent class analysis(LCA)was used to explore the latent classes of the symptom characteristics of PICS patients. Single factor analysis and multiple logistic regression were used to identify the influencing factors of the latent classes of PICS. Results A total of 299 samples were obtained and PICS occurred in 165 patients(55.18%). The symptom characteristics of PICS could be divided into 3 latent classes,which were named as “fatigue-sleep disorder group”(44.24%),“anxiety group”(16.97%),and “high symptom group”(38.79%). Compared “high symptom group” with “fatigue-sleep disorder group”,the higher acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score was,the less likely it was to be classified as “fatigue-sleep disorder group”(OR=0.882,P=0.001),and the more likely it was to be classified as “fatigue-sleep disorder group” without respiratory disease history or tracheotomy(OR=5.443,P=0.048;OR=4.015,P=0.006). Compared “high symptom group” with “anxiety group”,the higher APACHE Ⅱ score was Less likely to be classified to be “anxiety group”(OR=0.903,P=0.027),and patients younger than 50 years old were more likely to be classified as “anxiety group”(OR=3.392,P=0.025). Compared with “fatigue-sleep disorder group”,patients with age younger than 50 years were more likely to be classified as “anxiety group”(OR=4.422,P=0.005). Conclusion The symptomatic characteristics of PICS are heterogeneous and can be divided into 3 latent classes. Patients with a high APACHE II score,a history of respiratory disease,and a tracheotomy were more likely to be classified as “high symptom group”,while those younger than 50 were more likely to be classified as “anxiety group”. It is suggested that the clinical staff should pay attention to the heterogeneity of PICS individual symptom characteristics and give targeted intervention measures to different classes of patients.

This article reviews the latest studies on health related quality of life(HRQOL) in adult ICU discharged patients,including the short- and long-term status quo of HRQOL,influencing factors,and interventions. At present,the short- and long-term HRQOL in adult ICU discharged patients still remains unsatisfactory,as there are many influencing factors of HRQOL and the great individual differences of ICU discharged patients. It is suggested that personalized nursing interventions should be carried out in clinical practice aiming at the influencing factors,and effective interventions should be further explored to improve HRQOL of adult ICU discharged patients,so as to provide bases for the construction of systematic intervention plans.
